Documents You Need to Provide

Section A: Personal Data:	Documentation Required:
If your citizenship status is “protected person”	A photocopy of your Protected Persons Status Document.
If your citizenship status is “permanent resident”	A photocopy of your Permanent Resident Card.
If you declared yourself as a student with a permanent disability	Proof of your disability. Examples include: • a medical certificate, • a report or assessment (e.g., a learning disability assessment) completed by a qualified practitioner, or • documentation proving you are receiving federal and/or provincial disability assistance (e.g., ODSP)
If you have dependent children living with you full-time during your study period.	Proof of the age of each of your dependent children
If you previously received a Grant for Part-Time Studies	A copy of the transcript of your marks that proved you successfully passed the course(s) that you received the grant.
Section B: Income and Asset Information:	Documentation Required:
Your income	Proof of your current income (e.g. a letter from your employer or a photocopy of your recent cheque stub or social assistance stub)
Your spouse/common-law’s income (if you are married or in a common-law relationship)	Proof of your spouse/common-law’s current income (e.g. a letter from his/her employer or a photocopy of his/her recent cheque stub or social assistance stub)
Section D: Program Information:	Documentation Required:
If the school you are planning to attend is located outside of Canada	A photocopy of your Social Insurance Number card.
