Your OGS Application Package
The following is a list of items that make up students' OGS Application Packages. It will be personalized for you after you submit your online application. You are responsible for ensuring that all of the items in your personalized OGS Application Package are printed and submitted by the appropriate deadline date.
- Copy of your personalized OGS application and Copy of your Notice, Consent, Declaration and Signature of applicant (Required)
You MUST PRINT a copy of your completed online application. You can print this application right after you submit your online application, or you can print it at any time from your OGS Online Services Page. - Student Checklist (Required)
- Summary Sheet (Required)
If you are currently enrolled in an Ontario university or you have graduated from an Ontario university at any time between November 18, 2008, and November 18, 2009, your departmental ranking committee will complete items1 - 4 and sign the Summary Sheet. You must complete items 5 - 10 and submit this form to the department in which you are (were) registered at (immediately before) the time of application. In addition, your current department will also complete a departmental ranking sheet and submit it with your file to the ministry.
If you are not currently enrolled in an Ontario university (i.e., you are a direct applicant), you must complete items 5 - 10, sign the Summary Sheet and include with your application package. - Two Academic Assessment Reports - Fillable Form (Required)
You are limited to attaching one page, which must be signed by the professor. This page must be on letter-size paper, typed with single spacing, 12 point Arial font, and 1/2 inch margins. The Ministry has provided a PDF fillable form which may be used by the person providing the reference who wishes to elaborate on their assessment. This form is to be attached to the Academic Assessment Report and signed by the person providing the reference. If this form is used, comments must by typed directly on this page. The use of the Report is mandatory. Other forms or letters will not be accepted as substitutes. - Statement of Interest OR Plan of Study

Statement of interest is for for all applicants who will be enrolled in master's level studies during the tenure of the award.
Plan of study is for all applicants who will be enrolled in doctoral-level studies during the tenure of the award.
You must provide a one-page statement of interest or plan of study. The statement of interest should include your reasons for pursuing a graduate program in your chosen field. If you wish to include bibliographies or references these must be added to your plan or statement of interest because additional pages will not be accepted. The statement or plan must be provided on letter-size paper, typed with single spacing, 12 point arial font, and 1/2 inch margins. Any document that does not meet these standards will not be accepted.
- List of Scholarships and Awards (if applicable)

If you wish, you may provide a one-page list of scholarships and awards. This list must be provided on letter-size paper, typed with single spacing, 12 point arial font, and 1/2 inch margins. Any document that does not meet these standards will not be accepted.
- List of Significant Academic Accomplishments (if applicable)

If you wish, you may provide a one-page list of significant academic accomplishments (e.g., teaching/research assistantships, scholarly publications [differentiate between refereed, non-refereed, etc.], patents, conference presentations, and other evidence of scholarly activities, etc.). This list must be provided on letter-size paper, typed with single spacing, 12 point arial font, and 1/2 inch margins. Any document that does not meet these standards will not be accepted.
Note: If you consider that you have special circumstances, you may add a maximum of 100 words at the bottom of the Significant Academic Accomplishments page to describe any special considerations such as health problems, family responsibilities, disabilities, or other circumstances that have had an effect on your performance or productivity, including any delays in disseminating your research results or completing your degree.
The weight to be given to any special circumstances will be determined by the OGS panels evaluating your application. - Transcripts (Required)
- OGS requires official/original transcripts from studies done in Canadian and American Universities. No certified photocopies will be accepted. Print-outs from a website will not be accepted.
- OGS will accept copies or photocopies of transcripts from foreign universities where it is not possible to get another official transcript. However, the transcripts must be signed by the department or Graduate Studies office the student is applying through.
